This includes leave of absence (LOA) forms, short-term and long-term disability paperwork, return-to-work clearances, and other required documentation. Our approach ensures that forms are completed accurately, professionally, and in alignment with clinical findings, helping to support your needs while meeting employer or insurance requirements.
Nursing Home & Long-Term Care Services Uplift Health Clinic partners with nursing homes, assisted living facilities, and long-term care communities to provide comprehensive behavioral health services to residents. Services include psychiatric evaluations, ongoing medication management, and collaboration with interdisciplinary care teams. We address a range of conditions commonly seen in older adults, including depression, anxiety, mood disorders, cognitive-related behavioral disturbances, insomnia, and serious mental illness such as schizophrenia and bipolar disorder. Our goal is to improve quality of life, enhance emotional well-being, and support both residents and facility staff through consistent, compassionate care.
